When you choose LindenGrove Communities, you can be assured you or your loved one will receive the right amount of care regardless of how your needs may change. As a result, our continuum of care offers everything from temporary assisted living to 24/7 skilled nursing care.
Short-term rehabilitation services include physical therapy, occupational therapy and speech therapy.
Temporary Assisted Living
Our Bridge-to-Home apartments are designed for people recovering from a surgery, injury or illness who no longer need 24-hour skilled nursing, but still seek temporary support until they are ready to return home safely.
Our RCAC Assisted Living apartments provide supportive care so our residents can continue to live an independent lifestyle. One hour of personal assistance per day is included in the monthly rent, which may include a combination of medication management, personal care, housekeeping, bathing needs, and more.
Advanced Assisted Living
Our CBRF advanced assisted living suites, called the Lodges, provide enhanced assisted living support for adults living with advanced physical needs. These suites are designed for people who are in need of a higher level of care than RCAC assisted living apartments offer, but do not require 24/7 skilled nursing.
Our CBRF memory care homes are designed to meet the specific needs of a person with Alzheimer's disease, dementia or other type of memory loss.
Our skilled nursing services provide 24/7 skilled nursing care to meet the needs of a person who requires long-term care.
The Benefits of Continuum of Care
