LindenGrove Communities Expands Bridge-to-Home Program to Mukwonago
LindenGrove Communities has expanded its unique Bridge Program to its Mukwonago campus. The program provides temporary assisted living and health recovery support in a fully furnished apartment to adults who are recovering from an illness, injury or surgery.
The program was started earlier this year at the Waukesha campus as a way to provide support to the population of people who felt they still needed a little time before they could return home and live independently, yet no longer needed costly 24-hour-per-day skilled nursing care.
The new program solves this dilemma. Upon discharge from a hospital, emergency room or rehabilitation center, the individual moves into a fully furnished LindenGrove Communities apartment complete with a no-barrier bath and shower, kitchenette and personal storage, and receives health recovery support that might include rehabilitation services, daily living activity assistance, medication reminders and monitoring and medical appointment transportation until they are ready to return home. Plus, residential staff and a registered nurse are always on-site to ensure a safe, stress-free environment.
The program is intended for adults 62 years of age and older who intend to return to independent living after their short-term stay, which may be anywhere from a few days, weeks or months.
Because the apartment stay is temporary, there isn’t a long lease to sign. Instead residents in the program pay a daily rate, which ranges from $165 to $215.
