LindenGrove Communities is a local, nonprofit healthcare organization in Waukesha County offering short-term rehabilitation, skilled nursing facilities/nursing homes, assisted living apartments, and small-scale memory care homes. You can find LindenGrove Communities campuses in Menomonee Falls, Mukwonago, New Berlin, and Waukesha. LindenGrove Communities is known in the community and within the healthcare provider network for the delivery of high quality care for those in need of short-term rehabilitation following a surgery, acute illness, injury or stroke; or long-term skilled nursing care. LindenGrove Communities is governed by a local Board of Directors

Dedicated to Continuous Quality Improvement

Among the factors that set LindenGrove Communities patient and resident care apart from others is the high level of effort by staff to exceed national standards of practice. We are more than the average nursing home. The LindenGrove Communities Clinical Quality Assurance Department continuously monitors our quality of care to modify and improve processes, to ensure all state and federal regulations are understood, and to strive for continual compliance. Our staff is experienced, knowledgeable and well-trained. We promote consistent resident and patient staff assignment, which results in resident/care-giver bonds that build trust and compassion. Our Restorative Care Program is based on current standards of practice with the goal to assist residents and patients in reaching and maintaining their highest level of function. LindenGrove Communities has a culture of collaboration, excellence, openness and dedication.


Nursing Staff Is Certified. In-service Training Is a Priority.

Our nursing staff members are highly skilled in medical, surgical, and geriatric healthcare delivery. Nurses are credentialed to care for patients who require preventive and actual wound healing interventions. LindenGrove Communities diligently conducts in-service trainings for nursing staff members to ensure our level of care is excellent within our nursing facilities. It is important to note that the occurrence of pressure wounds that begin at our facility falls well below the national average. This is due to our resident- and patient-focused care and preventive measures. Our qualified nursing staff is the bridge to coordinated care among you, your family, and your personal physician.


Rehabilitation Nursing Services Help Patients Heal and Return Home

LindenGrove Communities is proud to provide rehabilitation nursing services to our patients and residents by credentialed staff members. The therapy goals of our patients and residents are consistently focused on returning the patient home in a timely manner or to offer superior nursing care as a resident of LindenGrove Communities on one of our four campuses.


Partnering with Local Colleges & Healthcare Institutions to Train Students

Local healthcare and education institutions throughout the greater Milwaukee area recognize the high quality of care provided at LindenGrove Communitiesand they send their students to our campuses to learn. Waukesha Family Medicine Residency Program in conjunction with the Medical College of Wisconsin and Waukesha Memorial Hospital, and St. Joseph's Family Medicine Residents Program in conjunction with the Medical College of Wisconsin and Wheaton Franciscan Healthcare partner with LindenGrove Communities where medical residents work with the medical director at LindenGrove Communities to complete their clinical rotation. LindenGrove provides the clinical practice site for Cardinal Stritch University, Carroll University, and Waukesha County Technical College (WCTC) for nursing and certified nursing assistant students. LindenGrove provides a practicum site that demonstrates to students top-of-the-line, high quality patient care. Multiple universities and colleges have students studying rehabilitation specialties and dietary internships at LindenGrove Communities. Healthcare students see that what is taught in the classroom is practiced daily at LindenGrove Communities. 


Electronic Medical Record System Aids Communication

LindenGrove Communities has an Electronic Medical Record (EMR) documentation system in each of our four skilled nursing facilities/nursing homes. Benefits of using the EMR system include: streamlined workflows, reduction of documentation duplication, and increased efficiencies. Access to improved communication with other healthcare providers is on the horizon. Each of the four LindenGrove Communities campuses is counted as 4 of the 5 Eden Alternative Registry Members in Wisconsin. We were among the first long-term care facilities in the nation to adopt the 10 core principles of The Eden Alternative. We persistently put resources into inservicing staff and continually integrating The Eden Alternative principles of residents making personal choices to direct care.

By practicing The Eden Alternative principles, LindenGrove Communities does not have an institutional nursing home feel. We strive to replace any sense of boredom, loneliness, and helplessness by creating environments that include animals, plants and gardens, intergenerational volunteers, and children. Our residents on all four campuses enjoy companionship, variety, and spontaneity.
